How we verify settlements

Eligible WPM Pathology data-breach class members can request $45 instead of other cash benefits, or claim qualifying documented losses and time spent responding to the breach. Submit online or by email by October 19, 2026; mailed forms must be postmarked by that date.

Two years of CyEx Identity Defense Complete monitoring is also available by request. Payment requires a valid claim and final approval; the defendants deny wrongdoing. This is not an automatic $4,000 payment.

Who qualifies

  • Your private information was compromised in the November 2024 breach, including people sent individual breach notices by WPM Pathology Laboratory or Salina Regional Health Center. Being a patient alone does not establish eligibility.
  • The court excludes officers and directors of WPM Pathology Laboratory and Salina Regional Health Center, or their respective subsidiaries or affiliated companies; governmental entities; the judge, the judge’s immediate family and court staff; and valid opt-outs. The court’s certified definition governs over the abbreviated FAQ list.
  • Losses must fall between November 4, 2024 and October 19, 2026 and remain unreimbursed. Extraordinary losses must be more likely than not caused by the breach, not already covered as ordinary expenses, with reasonable efforts to prevent or recover the loss.

How to submit your claim

  1. 1

    Choose your benefits

Choose $45 instead of all other cash benefits, or request qualifying ordinary expenses, extraordinary losses and described lost time. Monitoring can accompany either cash choice.

  2. 2

    Complete the official form

Online filing uses the LoginID and PIN from your notice. Ask Simpluris for missing credentials. The paper form asks for Notice ID if known, contact information, payment selection and the class member’s personal signature on the perjury attestation. Attach third-party records for losses; personal notes alone are insufficient.

  3. 3

    Submit by October 19

File through the official site, email an image of the completed form and documentation to Info@WPMPLDataBreach.com, or mail to WPM Pathology Data Breach Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958. Online/email submission is due October 19; mail requires an October 19 postmark. No exact hour or timezone was established.

  4. 4

    Keep your records

Keep a copy and respond to verification requests. Sign truthfully; this guide does not submit a claim or replace your certification. Questions go to Simpluris at 866-675-3062.

Frequently asked questions

Can I combine $45 with loss reimbursement?

No. The $45 choice replaces ordinary expenses, extraordinary losses and lost-time payments. Monitoring may accompany either choice.

What documents do I need?

Loss claims need receipts or other third-party evidence. Self-written explanations may support those records but cannot replace them. Lost-time claims need a brief description and attestation.

Is the maximum payment guaranteed?

No. The administrator must validate the benefit requested. Do not add maxima to imply everyone receives that amount.

Do I need a notice number?

Online entry requires LoginID and PIN. The paper form asks for Notice ID if known. Contact the administrator if you are unsure of eligibility or missing credentials.

When will payments arrive?

Benefits depend on final court approval and resolution of appeals. The hearing is scheduled for December 16, 2026; no payment arrival date is guaranteed.
