Who qualifies
- You may qualify if your private information was potentially impacted in the Baptist Health data incident involving unauthorized network access from approximately April 22 through July 8, 2024. The class includes those who received an incident notice. Being a Baptist Health patient alone does not establish that your information was affected.
- Excluded are Baptist Health and its affiliates, parents, subsidiaries, directors, officers and agents; governmental entities; assigned judges, their immediate families and court staff; anyone who perpetrated the incident; and people who validly opt out.
- For lost time, describe the actions you took in response to the incident and the hours spent, up to four. Attest truthfully under penalty of perjury. This benefit can be combined with one documented-loss option and credit monitoring.
- Choose ordinary expenses OR identity-theft/fraud losses, not both. The current form covers losses from April 22, 2024 through December 8, 2026. Losses must be actual, documented, unreimbursed and related to the incident. Provide supporting records; a personal statement alone is insufficient.
- For identity-theft or fraud losses, show that the incident more likely than not caused the loss and that you tried to prevent or recover it, including through available insurance. Do not claim the same expense twice or claim amounts reimbursed elsewhere.
How to file a Baptist Health settlement claim
- 1
Find your notice and select your benefits
Online filing asks for the Login ID and PIN from your notice. The paper form lists Notice ID as optional if known. If you need help identifying your notice or class membership, contact Simpluris at 866-606-0682. Select monitoring, lost time and one documented-loss option as applicable.
- 2
Gather records and sign your claim
Describe your incident-related time and attach records supporting any expenses or fraud losses. Complete the contact fields and personally sign and date the certification under penalty of perjury. The administrator may request additional evidence or verify your signature. ClaimBee helps you find claims and track deadlines; you remain responsible for your truthful declaration. Submit only one claim per person and request payment to yourself.

- 4
Keep your confirmation and watch for updates
Keep a copy of your claim and mailing or submission confirmation. Tell the administrator if your contact information changes and respond to any deficiency notice. Monitoring must be selected and activated using the instructions you receive. Final approval and claim validation must occur before benefits are distributed; no arrival date is guaranteed.
Online and mailed claims have different payment options
The current claim form says an approved mailed claim is paid by paper check to the address on the form. Digital payment choices are available through the online claim process. Follow the form's payment instructions when deciding how to submit, and keep your contact information current with the administrator.
The online portal requires the Login ID and PIN from your notice. If you cannot find them, ask the administrator for help. The paper form includes a Notice ID field marked 'if known'; this does not remove the requirement to belong to the settlement class or to sign a truthful claim.

Frequently asked questions
How much can I get from the Baptist Health settlement?
Eligible claimants can request up to $80 for lost time, plus either up to $400 in ordinary documented expenses or up to $4,000 in identity-theft or fraud losses. Two years of one-bureau credit monitoring is separately available. You cannot combine the $400 and $4,000 loss categories, and none of the maximums is an automatic payment.
What is the Baptist Health claim deadline?
The deadline is December 8, 2026. Online claims must be submitted by 11:59 p.m. Eastern Time; mailed forms must be postmarked by December 8. Use the official form and retain your confirmation or mailing record.
Which Baptist Health data incident does this cover?
This settlement concerns the 2024 incident at Baptist Health Medical Center–Drew County in Arkansas. The class covers people whose private information was potentially impacted, including notice recipients, subject to the stated exclusions. It does not cover every patient of every organization named Baptist Health.
Do I need receipts for the $80 lost-time benefit?
The lost-time claim requires a description of incident-related actions and time spent, plus an attestation under penalty of perjury. It pays $20 per hour for up to four hours. A separate expense or fraud-loss claim requires supporting documentation; your own statement alone is insufficient.
Can I claim both $400 and $4,000?
No. Ordinary documented expenses up to $400 and identity-theft or fraud losses up to $4,000 are alternatives. You may request lost-time compensation and monitoring alongside the applicable loss option, but cannot recover an expense already reimbursed by another source.
Do I need a Login ID and PIN?
The online portal asks for the Login ID and PIN from your notice. The paper form asks for Notice ID if known, but still requires your contact details and signed certification. Contact the administrator if you are unsure whether you belong to the class; mailing a form does not establish eligibility.
When will Baptist Health settlement payments arrive?
No payment arrival date is guaranteed. The settlement needs final approval and must become effective, and your claim must be validated. A scheduled court hearing is not a payment date. Follow the administrator's updates and respond promptly to requests for missing information.
