Last updated: October 14, 2026
The Brockton Neighborhood Health Center settlement offers eligible people up to $50 in alternative cash or up to $2,500 for qualifying losses and time spent responding to its November 2024 data incident. Payments may be reduced. Submit your claim online or mail it postmarked by December 8, 2026.

Who qualifies
- You resided in the United States and your private information was impacted in the data incident affecting Brockton Neighborhood Health Center around November 1–3, 2024. The class includes people who received a notice of the incident.
- The defendant, the assigned judges and their staff and families, and people convicted of or pleading no contest to specified criminal involvement in the incident are excluded. Valid opt-outs cannot receive settlement benefits.
- The loss option covers qualifying unreimbursed monetary losses caused by the incident between November 1, 2024 and December 8, 2026. Losses must have been denied reimbursement and cannot be part of a pending third-party reimbursement request.
- Provide third-party records for monetary losses. Self-prepared records alone are insufficient. For time spent responding to the incident, attest to the actual time claimed; up to three hours at $30 per hour counts toward the same $2,500 limit.
- Choose the alternative cash payment instead of the loss/time option if appropriate. It starts at up to $50, needs no supporting loss documents and can be reduced. You still must qualify and submit a truthful signed claim.
How to file a Brockton data settlement claim
- 1
Check your notice and membership
Review the official class definition and your breach or settlement notice. Being a patient alone does not establish that your information was impacted. Ask the administrator if you are unsure whether you are included.
- 2
Choose your benefits
Select the loss/time option or alternative cash, then request monitoring if you want it. Include third-party records for financial losses and attest to actual time. Do not add the $50 alternative payment to a loss/time claim.
- 3
Complete and sign the official form
The online form requires the Class Member ID from your settlement notice. If it is missing, ask the administrator for help. You can also download the paper form. Review all information and sign your own declaration. An authorized representative filing for a minor or deceased class member must provide documents proving that authority.

File by December 8, 2026
Submit online or mail your completed form and required supporting documents postmarked by December 8. Use the address printed on the form and keep a submission or mailing record. Paper claims pay by check; use online filing for electronic payment options.

Frequently asked questions
How much does the Brockton settlement pay?
Eligible claimants can choose up to $50 alternative cash or up to $2,500 for qualifying monetary losses and time responding to the incident. These are limits, not guaranteed checks. Payments may decrease to fit the settlement's overall cap after approved deductions.
Can I add $90 for time to a $2,500 loss claim?
No. Brockton's time benefit is $30 per hour for up to three actual hours, or $90 maximum, and is included in the $2,500 combined limit. You must attest to the time spent responding to the incident. The alternative $50 cash option replaces the loss/time option.
Why could the $50 payment be reduced?
The settlement's $825,000 cap includes approved fees, costs and administration. If funds are insufficient, the agreement pays approved benefits in this order: credit monitoring, documented monetary losses, lost time, then alternative cash. Proportional reductions apply as necessary. The $50 alternative is not guaranteed.
Do I need receipts to claim?
Third-party records are required for financial losses; a handwritten explanation alone is not enough. Supporting loss documents are not required for alternative cash, and time claims require an attestation rather than receipts. All claims require truthful information, a signature and administrator verification.
Can I get credit monitoring as well as cash?
Yes. Participating class members may request three years of one-bureau credit monitoring in addition to either cash option. You must request the benefit and follow the activation instructions sent after final approval. Monitoring is not automatically activated by filing a claim.
What is the Brockton claim deadline?
Submit your official claim form online or mail it postmarked by December 8, 2026. Include the required signature, financial-loss evidence if applicable, and proof of authority if filing for a minor or deceased class member.
When will Brockton settlement payments arrive?
No specific arrival date is promised. Payments depend on final approval, the settlement becoming effective and approval of your claim. Use the administrator's website for updates and keep your contact information current.
