DaVita ransomware settlement: claims due January 19
Last updated:
Sources last checked:
Eligible DaVita settlement members may claim an estimated $50 cash payment, up to $2,500 for qualifying documented losses, and three years of credit monitoring. You may choose any or all three benefits. Submit a valid claim online or by mail postmarked by January 19, 2027.
The settlement covers U.S. people whose personal information was potentially accessed without authorization in the April 2025 ransomware attack. Being a DaVita patient alone does not establish eligibility. DaVita denies wrongdoing, and benefits depend on final court approval.
The $50 figure is an estimate that can rise or fall. Loss reimbursements can also be reduced. The settlement provides a $10 million fund plus a separate supplemental amount of up to $5 million; it does not promise every claimant $2,550.

Who qualifies
- You must be a U.S. person whose personal information was potentially accessed without authorization in the DaVita ransomware attack in April 2025. Check your notice or ask the official administrator to confirm your membership.
- Excluded are DaVita, entities it controls, its affiliates, parents, subsidiaries, officers, directors, legal representatives, successors and assigns; judges or judicial officers overseeing the case and their immediate families and staff; and anyone who validly opts out.
- For loss reimbursement, the loss must be actual, documented, unreimbursed and fairly traceable to this attack. It must have occurred from April 11, 2025 through January 19, 2027; only include expenses already incurred by the date you complete your claim.
- You must have made reasonable efforts to avoid or recover the claimed loss, including disputing unauthorized transactions and exhausting existing credit-monitoring or identity-theft insurance. You must certify the information under penalty of perjury.
How to claim DaVita settlement benefits
- 1
Find your notice credentials
The official online claim process asks for your Unique ID and PIN. If your notice is missing, contact the administrator at 1-877-239-8791. A downloadable paper form is also available.
- 2
Choose your benefits
Select the pro rata cash payment, three-year credit monitoring, documented-loss reimbursement, or any combination. A timely valid claim is required even if you only want monitoring.
- 3
Document any losses
Provide the amounts, dates, descriptions, supporting records and explanation linking each loss to the attack. Receipts or other records not prepared by you can support a claim; your own handwritten receipt alone is insufficient. Do not claim an expense already reimbursed elsewhere.

How the settlement funds work
The $10 million fund pays approved administration costs, taxes, legal fees, service awards and claimed monitoring. The remainder funds pro rata cash payments. A separate supplemental amount covers valid documented losses and any supplemental attorney fees, with a combined $5 million cap; loss payments can be reduced if the cap is exceeded. These totals include costs and fees, not only cash to class members.

Frequently asked questions
Is the DaVita settlement payment guaranteed to be $2,550?
No. The pro rata cash amount is currently estimated at $50 and can increase or decrease. Up to $2,500 of qualifying loss reimbursement is a separate conditional benefit that requires proof and can be reduced. You may claim both, but there is no guaranteed $2,550 payment.
Do I need receipts for the estimated cash payment?
The pro rata cash option does not require a documented monetary loss. You still must be an eligible class member and submit a valid signed claim. Loss reimbursement requires supporting records, an explanation and the required certification.
Can I get credit monitoring without filing a claim?
No. You must submit a timely valid claim and select monitoring. It provides three years of one-bureau monitoring, dark-web monitoring, identity-theft insurance coverage up to $1 million and identity recovery services.
What if I cannot find my Unique ID or PIN?
Contact the settlement administrator at 1-877-239-8791. The online form requires both credentials from the notice. The official documents page also supplies a paper claim form; follow the administrator’s directions if information is missing.
What is the DaVita claim deadline?
The official claim form and current court order set January 19, 2027. File online by that date or mail your form with a January 19 or earlier postmark. No precise online cutoff time or timezone was stated in the reviewed sources.
How will approved payments be delivered?
The paper form defaults to a mailed check and also offers PayPal, Venmo, Zelle or ACH. Choose only one payment method and supply details only through the official process. The form says selecting multiple options results in a mailed check.
When will DaVita settlement benefits arrive?
Benefits will be distributed after court approval and the settlement becomes final. The final approval hearing is scheduled for February 18, 2027, at 10 a.m.; the schedule can change. No fixed payment date is confirmed.
