People whose private information was accessible in Wayne Memorial Hospital’s May 30–June 3, 2024 data incident may claim one of two cash options: reimbursement of qualifying documented losses related to fraud or identity theft up to $5,000, or an estimated $25 alternative payment that may be reduced. Eligible class members also receive a code to enroll in two years of credit monitoring.
Submit an online claim by December 7, 2026 at 11:59 p.m. Eastern Time, or mail a claim postmarked by December 7. Cash payments require a valid claim, final court approval and finality. Wayne Memorial denies wrongdoing. ClaimBee provides an independent reference; the official administrator handles claims.

Who qualifies
- The class includes people whose private information was accessible because of the data incident at Wayne Memorial Hospital Auxiliary, Inc., doing business as Wayne Memorial Hospital, from approximately May 30 through June 3, 2024. A breach notice can help identify affected people; merely being a hospital patient does not establish eligibility. The class definition does not limit membership to Georgia residents.
- Excluded are the defendant’s directors and officers, governmental entities, the assigned judge, the judge’s immediate family and court staff, and anyone who timely opts out. Contact the administrator if you are unsure whether your information was affected.
How to claim a cash payment
- 1
Locate your notice credentials
For online filing, use the CPT ID and Passcode in your notice at the official claim login. If either is missing, email WMHDataSettlement@cptgroup.com or call 1-888-716-0934. The paper form asks for your CPT ID and contact details. Do not assume eligibility from a public article.
- 2
Choose one cash option
For Cash Payment A, list your losses, dates and amounts and attach supporting records. Losses must relate to fraud or identity theft, be actual and unreimbursed, more likely than not caused by the incident, incurred on or after the incident, and subject to reasonable but unsuccessful efforts to avoid them or obtain reimbursement. For Cash Payment B, choose the estimated $25 alternative; no loss documentation is required.
- 3
Sign the claim
Provide the requested contact information and sign the declaration under penalty of perjury. The administrator may request additional information. Self-prepared documents alone do not establish a documented loss, though they may explain other records. Do not claim expenses already reimbursed by another source.

Submit by December 7

Frequently asked questions
Can I receive both cash payments?
No. The detailed settlement terms and claim form make Cash Payment B an alternative to Cash Payment A. Choose documented-loss reimbursement up to $5,000 or the estimated $25 alternative.
Is $25 guaranteed?
No. It is an estimate. Cash Payment B awards are limited to $300,000 in total and are reduced proportionally if that cap is exceeded. The $300,000 figure is the cap for alternative cash awards, not the total value of every settlement benefit.
What proof is required?
Documented-loss claims need reasonable supporting records such as bank or credit-card statements, invoices, telephone records or receipts. Personal statements or handwritten receipts alone are insufficient. The alternative cash option does not require loss documentation, but eligibility, notice credentials, a truthful signed claim and possible administrator verification still apply.
Do I need to claim credit monitoring?
No cash claim is required to receive the enrollment code. Class members are entitled to two years of single-bureau monitoring through CyEx or an equivalent provider, with $1 million in identity-theft insurance. The code must be activated after final approval; receiving a code does not automatically enroll you. Follow the official enrollment instructions.
When will payments arrive?
No payment date is guaranteed. The notice schedules a final approval hearing for January 27, 2027 at 1:30 p.m. Eastern Time. Benefits are distributed after approval and finality, which may be delayed by appeals or claim processing. Check the official website for changes.
What happens if I do nothing?
You receive a credit-monitoring enrollment code but no cash payment. If the settlement becomes final, class members who did not opt out are bound by its release. The notice lists November 7, 2026 for exclusion requests and objections. These use separate procedures; read the notice and signed court order for the full requirements.
