How we verify settlements

People residing in the United States or its territories whose private information was determined to have been potentially impacted by the Jefferson-Blount-St. Clair Mental Health Authority data incident may claim benefits. This includes people sent an incident notice; being a patient or employee alone does not establish eligibility.

Choose a documented-loss cash claim of up to $5,000 or a $100 cash alternative, with optional three years of credit monitoring. All cash payments can increase or decrease proportionally. File online by 11:59 p.m. Eastern Time on December 23, 2026, or mail a claim postmarked by that date.

Who qualifies

  • You must reside in the United States or its territories and have private information determined to have been potentially impacted by the JBS data security incident on or around November 25, 2025. People sent a notice letter are included. Ask the administrator if you are unsure.

How to submit a claim

  1. 1

    Locate your notice credentials

The online form requires your CPT ID and Passcode. The paper form asks for your CPT ID. If you lack the credentials, contact CPT Group at 1-888-716-0970 or JBSSettlement@cptgroup.com.

  2. 2

    Choose cash and monitoring benefits

Select either documented losses up to $5,000 or the $100 cash alternative. You may also request three years of credit monitoring, including without a cash election. Amounts are conditional and cash is subject to proportional adjustment.

  3. 3

    Document losses and sign

For losses, provide dates, amounts and reasonable third-party records. Losses must be actual, unreimbursed, incurred on or after the incident and more likely than not caused by it; the form also asks for reasonable unsuccessful efforts to avoid or obtain reimbursement. Self-prepared documents alone are insufficient. Sign and date the penalty-of-perjury certification. The administrator may request additional information.

  4. 4

    File by December 23

Use the official website’s Submit Claim link by 11:59 p.m. Eastern Time on December 23, 2026, or mail the completed signed claim and supporting records postmarked by December 23 to Jefferson-Blount-St. Clair Data Security Incident, c/o CPT Group, Inc., PO Box 19504, Irvine, CA 92623. Frequently asked questions

Is the $100 payment guaranteed?

No. The $100 alternative may increase or decrease based on valid claims and the available net fund. The agreement applies equal-percentage proportional adjustments to all cash payments, including documented-loss payments.

Can I claim both cash options?

No. Choose documented losses or the $100 alternative. Monitoring can accompany either choice or be requested without cash. If loss documentation is missing, or a rejected loss claim is not cured, the claim is treated as the cash alternative.

What documents support a loss claim?

Examples include bank or credit-card statements, invoices, telephone records and receipts. Unrelated transactions may be redacted. A handwritten receipt, declaration or other self-prepared document alone is insufficient, though it may clarify other reasonable records.

Do cash-alternative and monitoring claims require loss receipts?

The issued form does not request loss documentation for those options. You must still provide the requested claimant information and sign the certification; claims remain subject to verification and supplemental-information requests.

When will benefits arrive?

Benefits depend on final court approval, the settlement becoming final and validation of the claim. No fixed payment date is established. The current amended order schedules a February 16, 2027 hearing; a hearing is not approval.

What happens if I do nothing?

You receive neither cash nor monitoring. If you remain in the class, you are bound by the settlement and release if it becomes final. The notice lists November 23, 2026 for exclusion requests and objections; consult the notice for those separate procedures.
