- You are a person in the United States who used the appointment booking tool on livehealthonline.com, the LiveHealth Online iOS app or the LiveHealth Online Android app between October 2024 and August 2025.
- Having an Amwell account or visiting a page alone does not establish membership: the class definition requires use of the specified LiveHealth Online booking tool during the covered period.
- Excluded are the presiding judges and their families; Amwell and its specified related entities and their current or former officers, directors, agents, attorneys and employees; valid opt-outs; and the excluded parties’ legal representatives, successors and assigns.
