Who qualifies

  • You reside in the United States or its territories and Equifax's records place you in a covered group. Having an Equifax credit report alone does not qualify you.
  • Certified Group: Equifax received your mailed hard-inquiry dispute from October 4, 2016 through March 27, 2026, coded it as 'not mine' (001) or 'unauthorized' (383), and sent a document containing Cons Comm 664, the response describing inquiries as a factual record of file access. All these conditions apply.
  • General Dispute Group: Equifax's records show it received at least one hard-inquiry dispute from August 31, 2021 through March 27, 2026. The Certified Group's mailed-letter and response-code conditions do not apply to every General Dispute Group member.
  • New York Subclass: Certified Group members with a New York address who received the specified response during November 27, 2017–September 10, 2024. New York residency alone is insufficient. Your settlement notice identifies your group.
  • To be paid, truthfully certify harm from the dispute handling or a credit denial partly caused by the disputed inquiry, and sign your claim under penalty of perjury. People excluded by the court, including valid opt-outs, do not receive settlement benefits.

How to file an Equifax inquiry dispute claim

    Check your notice and group

    Find the Claim Number and PIN in your settlement notice. Compare your dispute history with the official class definition. If your notice is missing or you need a paper claim, contact the administrator at (833) 244-4150 about identity validation.

    Complete the official form yourself

    Use the official online form with your Claim Number and PIN. Complete the contact section and select at least one truthful harm statement: economic loss, privacy loss or other negative consequences from Equifax's handling, or a credit denial partly because of the disputed inquiry. Personally sign the certification.

    Submit by November 29, 2026

    Keep your submission confirmation. The agreement allows a paper form after identity validation with notice credentials; ask the administrator to complete that step and confirm the current return instructions. Do not assume a deadline-day postmark is sufficient: the paper form says it must be returned no later than November 29.

    Follow approval and payment updates

    Payments depend on final approval and resolution of appeals. A paper check is the default, with an electronic payment option through the administrator. Keep your contact information current; the December 10 hearing does not establish a payment arrival date.
