How we verify settlements

USC Arcadia Hospital agreed to a $200,000 settlement over allegations that tracking tools on its website shared users’ private information with Meta and other third parties. The hospital denies wrongdoing. Cash payments require a timely, valid claim and final court approval; no individual payment amount is guaranteed.

The class covers people who completed forms on the hospital’s website to sign up for email or event updates from February 1, 2022 through January 31, 2023, subject to the exclusions below. Being a hospital patient or simply visiting its website does not by itself establish eligibility.

Submit your cash claim online or mail a signed paper form postmarked by December 7, 2026. Eligible members can also activate one year of CyEx Privacy Shield Pro using their notice code by March 15, 2027; a cash claim is not required for that service.

Who qualifies

  • You completed a form on USC Arcadia Hospital’s website to sign up for email or event updates between February 1, 2022 and January 31, 2023. The stated definition does not impose a California-residency requirement.
  • Excluded are the judges presiding over the action and their families; the hospital, its subsidiaries, parents, successors and predecessors, entities controlled by it or its parents, and its current or former officers and directors.
  • People who submit a valid, timely exclusion request, and successors or assigns of excluded natural persons, are also excluded. Contact the settlement administrator if your membership is uncertain.

How to request a cash payment

  1. 1

    Check your eligibility and notice

Confirm that you completed a covered email/event signup. Your notice includes a LoginID and PIN for online filing. If you cannot find them, contact Simpluris at info@USCArcadiaHospitalSettlement.com or (844) 496-0567.

  2. 2

Submit online or use the paper form

Use the official claim page or download the three-page form. Provide your name, mailing address and contact details. Paper claims require your signature, date and printed name, certifying that the information is true and correct to the best of your knowledge. The administrator may request additional documents.

  3. 3

    Meet the December 7 deadline

Submit online by December 7, 2026, or mail the signed form postmarked by that date to USC Arcadia Hospital Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799. The reviewed instructions do not specify an exact online cutoff time or time zone.

  4. 4

Choose payment and keep contact details current

Valid paper claims receive a mailed check. To choose PayPal or Venmo, file online and select that payment type. Notify the administrator in writing at the mailing address above if your contact information changes before distribution. Payments depend on final approval and the settlement becoming effective.

  5. 5

Activate the separate privacy service if desired

Class members do not need a cash claim for one year of CyEx Privacy Shield Pro. Use the activation code and instructions in your notice by March 15, 2027. This activation date does not extend the December 7 cash-claim deadline.

Frequently asked questions

How much will I receive?

Approved cash claimants receive equal shares of the net settlement fund. The gross fund is $200,000; court-approved fees, costs, administration, taxes, any service award and privacy-service costs reduce it. The payment depends on the number of valid claims, so no fixed amount is promised.

Do I qualify just because I was a patient?

The stated class requires completing a hospital website form for email or event updates during February 1, 2022–January 31, 2023. Patient status or a website visit alone is not the stated test.

Do I need a receipt or proof of financial loss?

The paper form asks for claimant contact details and a signed truth certification, rather than a documented financial-loss claim. The administrator must validate the claim and may ask for additional documents. Online filing requires the LoginID and PIN from your notice.

Can I receive privacy services without claiming cash?

Yes. Eligible members receive a notice activation code for one year of CyEx Privacy Shield Pro, with activation due March 15, 2027. A timely cash claim is separately required to receive money.

What are the other deadlines?

The published exclusion and written-objection deadline is December 7, 2026. These procedures have separate requirements in the official notice. The final approval hearing is scheduled for December 14, 2026 at 10:30 a.m.; check the administrator’s website for changes.

When will payments arrive?

No guaranteed payment date is published in the reviewed instructions. Cash payments require final approval and the settlement becoming effective. The hearing date is not a distribution date.
