How we verify settlements

People whose private information was affected by the April 23–May 5, 2024 Palomar Health Medical Group data incident may claim benefits from a $3.1 million proposed settlement. Submit online by 11:59 p.m. Eastern on October 22, 2026, or mail a claim postmarked by October 22.

Choose either reimbursement for eligible documented losses, with a stated limit of $5,000, or an alternative cash payment estimated at $60. Both cash options are subject to proportional increases or decreases under the settlement; $60 is not guaranteed. Two years of single-bureau credit monitoring may be claimed separately or alongside one cash option.

The court granted preliminary approval on July 17, 2026. The final approval hearing is scheduled for November 6, 2026, at 10:30 a.m. Benefits depend on final approval and the settlement becoming effective. Palomar denies wrongdoing.

Who qualifies

  • Your private information was accessed, acquired, disclosed or compromised in the incident involving Palomar Health Medical Group systems between April 23 and May 5, 2024. Receiving healthcare alone does not establish inclusion.
  • The official class definition does not limit eligibility to residents of California or another state. It turns on whether your information was affected by this specific incident.
  • People who received the settlement notice may submit a claim. If uncertain, contact the administrator at 1-844-440-4203.
  • Excluded are directors, officers and agents of the defendant or its subsidiaries and affiliates; governmental entities; and the assigned judge, immediate family and court staff. People who validly opted out cannot receive benefits.

How to submit an official claim

  1. 1

    Confirm your inclusion

Review the official notice and your records. The online portal offers routes with a Notice ID and Confirmation Code or without a notice. The paper form asks for a Notice ID if known.

  2. 2

    Choose benefits and gather proof

Choose documented-loss reimbursement or alternative cash, and optionally credit monitoring. Loss claims need evidence of actual, unreimbursed expenses related to the incident; a personal statement alone is not sufficient. The alternative cash option does not require loss documentation. Sign and date the required truthful certification.

  3. 3

    File before the deadline

Use the free official claim portal or mail the signed form and copies of supporting documents to Palomar Health Data Incident Settlement, PO Box 60, East Brunswick, NJ 08816-9998. Online claims are due by 11:59 p.m. Eastern on October 22, 2026; mailed claims must be postmarked by October 22. Keep a copy.

Frequently asked questions

Can I receive both cash options?

No. Choose documented-loss reimbursement or alternative cash. Credit monitoring may be requested by itself or with one cash option.

Is $60 guaranteed?

No. It is an estimate. Cash payments can increase or decrease proportionally based on valid claims and the available net fund. The administrator determines whether each claim is valid. The fund pays administration, court-approved fees and awards, then claimed monitoring, then approved documented-loss claims, before alternative cash claims; the two cash options do not have equal payment priority.

What proof does a loss claim require?

Provide reasonable records supporting actual, unreimbursed losses related to this incident, such as receipts, phone records or correspondence. Personal certifications alone do not establish the loss. Attest under penalty of perjury and respond to any request for additional information.

What does credit monitoring include?

Two years of single-bureau monitoring, dark-web monitoring and $1 million in identity-fraud insurance. Provide an email address for the activation code after final approval and effectiveness. Insurance coverage is not a $1 million settlement payment.

When will benefits be paid?

No fixed distribution date is established in the reviewed notice. Final approval is scheduled for consideration on November 6, 2026; appeals and claim processing can affect timing.
