Who qualifies

  • You must reside in the United States, have private information potentially accessed or acquired in Holt Group’s December 26–27, 2024 data incident, and have been sent a letter notifying you about that incident. Being a Holt customer or employee alone does not establish membership.
  • Excluded are the defendant and its affiliates, parents, subsidiaries, directors, officers and agents; governmental entities; the assigned judge, the judge’s immediate family and court staff; people who validly opt out; and anyone who perpetrated the incident.

How to file a claim

  1. 1

    Check your notification

Use the official settlement website and the unique Login ID and PIN supplied with your notice. If you need help locating your information, contact Simpluris at 1-833-296-0885 or info@HoltGroupDataSettlement.com. The printable claim asks for a Login ID if known.

  2. 2

Choose your cash benefits and monitoring

Choose documented losses and/or lost time, or choose the alternative $50 cash payment instead of both. You may also request two years of three-bureau monitoring. For documented losses, the agreement requires actual, unreimbursed monetary losses related to fraud or identity theft, more likely than not caused by the incident. The claim form specifies December 26, 2024 through December 1, 2026.

  3. 3

    Provide evidence and sign

For documented losses, include receipts, account statements or other reasonable records and the amounts claimed. Personal certifications, declarations or affidavits alone are insufficient, though they may explain other evidence. Do not claim losses already reimbursed. For lost time, select up to four hours at $20 per hour and briefly explain the incident-related tasks. Alternative cash needs no loss documentation. Complete contact fields and personally sign the perjury certification; the administrator may request more information.

  4. 4

    Submit by December 1

Submit online through the official settlement website by 11:59 p.m. Eastern Time, or mail the completed signed form and supporting records postmarked by December 1, 2026 to Holt Group Data Incident Settlement, c/o Settlement Administrator, PO Box 25191, Santa Ana, CA 92799-9958. Submit only one claim. Mail claims are paid by paper check; online claims may offer digital payment options.

Frequently asked questions

Can I claim both documented losses and lost time?

Yes. The agreement permits documented-loss and lost-time claims together. Documented losses are capped at $5,000, and lost time at four hours at $20 per hour ($80). The alternative $50 replaces both of those cash categories.

Does the $5,000 option cover every expense after the breach?

No. Agreement paragraph 60(a) requires an actual, documented, unreimbursed monetary loss related to fraud or identity theft and more likely than not caused by the data incident. The notice’s examples of credit reports, monitoring, credit freezes, replacement IDs and postage do not remove those conditions. The claim form specifies losses during December 26, 2024–December 1, 2026.

Do I need receipts for the $50 payment?

No loss documentation is required for the alternative cash payment, but you must be eligible, submit the claim on time and sign the required certification. It cannot be combined with documented-loss or lost-time cash payments.

Can I receive monitoring with a cash payment?

Yes. Eligible claimants may request two years of three-bureau credit monitoring with at least $1 million in fraud protection in addition to their cash choice.

Is there a fixed fund shared among claimants?

The agreement provides benefits on a claims-made basis, with the defendant paying approved benefits, administration and court-awarded fees and service awards. It does not establish a fixed cash fund to divide proportionally among all claimants.

When will payments arrive?

The notice schedules a final approval hearing for December 16, 2026. Payments depend on final court approval and resolution of any appeals. A hearing date is not approval, and there is no guaranteed payment date.

What happens if I do nothing?

You will not receive settlement benefits without a valid claim. Unless you validly opt out, the settlement’s release can bind you if approved. The notice gives November 16, 2026 as the opt-out and objection deadline; see the official notice for the separate procedures.
