How we verify settlements

The proposed $2 million Covenant Health data settlement offers eligible class members documented-loss reimbursement based on up to $5,000 and a separate proportional cash payment estimated at $75. You may claim both. Cash amounts can increase or decrease; neither amount is guaranteed. Submit a cash claim online by December 30, 2026 at 11:59 p.m. Eastern Time, or mail it postmarked by that date.

The court-approved class definition covers living U.S. residents whose private information was compromised in the May 2025 data incident, including people sent a breach notice. Class members receive a code for two years of credit monitoring without filing a cash claim; enrollment becomes available after final approval and effectiveness.

Who qualifies

  • You are a living person residing in the United States whose private information was compromised in the data incident involving Covenant Health Inc., St. Joseph Healthcare Foundation, St. Joseph Hospital and St. Joseph Hospital Nashua, N.H. The court definition includes people sent a notice of the incident.
  • The lawsuit concerns unauthorized access to the defendants’ IT environment around May 18, 2025. Being a patient alone does not establish class membership; the compromised-information requirement applies.
  • Excluded are the defendants’ directors and officers, the judges assigned to the action and their immediate family members and court staff, and class members who validly opt out.

How to request a cash payment

  1. 1

    Confirm your eligibility

Read the official notice and class definition. If you are unsure whether you belong to the class or cannot find your notice, contact Kroll through the official settlement website.

  2. 2

Choose either or both cash benefits

Select documented losses, the proportional cash payment, or both. For losses, attach third-party records showing unreimbursed costs caused by the incident. The proportional payment does not require loss documentation.

  3. 3

Complete and personally sign the form

Supply your contact details, payment choice and Class Member ID if known. The claimant must personally sign the paper or electronic form under the required attestation. Keep copies of your claim and supporting records.

  4. 4

    Submit before the official cutoff

Use the official online form by December 30, 2026 at 11:59 p.m. Eastern Time, or mail the signed paper form and attachments postmarked by December 30. Filing with the administrator is free; a ClaimBee subscription is not required.

Frequently asked questions

Can I request both cash benefits?

Yes. The settlement agreement and current claim form allow documented-loss reimbursement and the proportional cash payment together, or either one alone. Both remain subject to claim validation and proportional adjustments.

Is the $75 payment guaranteed?

No. It is an estimate of the proportional payment from the remaining fund. Administration, approved legal fees and costs, service awards, monitoring and documented-loss payments affect the balance. The number and value of valid claims can change cash amounts.

What proof is needed for losses?

Provide reasonable third-party documentation such as receipts, invoices, statements or bills, and explain the connection to the incident. You cannot recover a cost already reimbursed elsewhere. Self-prepared notes alone are insufficient, though they may explain supporting documents. An uncured rejected loss claim converts to the proportional cash option under the settlement rules.

Do I need a claim form for credit monitoring?

No cash claim is required for the monitoring benefit. Class members receive an enrollment code on their postcard notice for two years of CyEx Medical Shield Complete. The code becomes usable after final approval and effectiveness. Contact the administrator if your code is missing.

When will benefits arrive?

No payment date is confirmed. The administrator has not listed a final hearing date. Benefits depend on final court approval, the settlement becoming effective and resolution of any appeals.
