How to claim a cash payment

  1. 1

    Download the official paper form

Use the official claim-form link below. Enter your name, mailing address, email and telephone number. The paper form asks for your Login ID if known. For help or a replacement notice, call 844-496-1265 or email info@PillsburyDataSettlement.com.

  2. 2

    Choose your cash benefits

You may request documented-loss reimbursement and a pro rata cash payment together. For losses, list the amounts and attach records showing actual, unreimbursed losses from fraud or identity theft caused by the incident. The issued form covers losses between April 29, 2025 and December 7, 2026. The agreement also requires reasonable efforts to avoid the loss or obtain reimbursement, including exhaustion of available credit-monitoring and identity-theft insurance.

  3. 3

    Sign and include supporting records

Select your payment method and sign and date the declaration under penalty of perjury. Bank statements or receipts can support a loss claim; personal notes alone are insufficient. Put your name and signature on any additional loss sheets. The pro rata cash option does not require loss documentation, but eligibility, a truthful signed claim and possible administrator verification still apply.

  4. 4

    Mail by the deadline

Mail the completed form and any supporting documents postmarked by December 7, 2026 to Pillsbury Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958. Submit only one claim per person and notify the administrator if your contact details change. The official notice also offers online filing; the agreement sets its cutoff at 11:59 p.m. Eastern Time on December 7.

Frequently asked questions

Can I receive both cash benefits?

Yes. A valid documented-loss claim may receive up to $3,000, and you may also claim a share of the remaining settlement fund. Neither the maximum reimbursement nor any particular cash-share amount is guaranteed.

How are the payments funded?

The agreement establishes a $2.34 million fund for pro rata cash payments, medical monitoring, administration and court-approved fees, costs and service awards. Pillsbury pays qualifying documented-loss claims separately, up to $1.5 million in total. If valid loss claims exceed that cap, those reimbursements are reduced proportionally. The amount of each pro rata cash payment is not yet known.

What losses and proof qualify?

Losses must be actual, documented, unreimbursed, fairly traceable to fraud or identity theft caused by the incident, and within the covered loss period. You must have made reasonable efforts to avoid the loss or seek reimbursement, including using available insurance. Submit records such as bank statements or receipts. Your own notes may explain other evidence but cannot establish the loss by themselves. Do not claim an expense already reimbursed elsewhere.

Do I need a cash claim for medical monitoring?

No. The agreement provides an activation code to class members separately from cash claims. The issued notice identifies one year of CyEx Medical Shield Complete, including $1 million in identity-theft insurance. Receiving a code does not automatically enroll you. The agreement ties activation to settlement effectiveness and cash distribution; follow the administrator’s instructions and contact it if your code is missing.

Will the administrator ask for more information?

It may verify your claim and request additional information or documentation. Under the agreement, a deficiency response is due by the later of the claim deadline or 10 days after the deficiency notice is sent. An incomplete, unsupported or untimely response may lead to a reduced or denied claim.

When will I receive money?

No payment date is guaranteed. The official notice schedules a final approval hearing for December 22, 2026 at 8:45 a.m. Eastern Time. Payments depend on approval, finality and claim processing, and appeals may delay them. Check with the administrator for changes.

What if I do not file a cash claim?

You will not receive cash, but the agreement separately provides medical-monitoring activation codes. If you remain in the class, the settlement’s release becomes binding when it takes effect. The notice lists December 7, 2026 for opt-out requests and objections. Opt-out requests must be postmarked by then; objections must be filed with the court and copied to the administrator and both sides’ counsel under the separate official procedures.
