Regional Urology and Ochsner LSU Health Data Settlement
Last updated:
Sources last checked:
Living individuals in the United States whose private information was potentially compromised in the October 5, 2025 data incident may claim settlement benefits by November 16, 2026. The incident involved information related to care at Regional Urology before December 31, 2022; receiving care alone does not establish class membership.
Eligible members may choose a $40 alternative payment, or request one year of CyEx Medical Shield Complete and/or reimbursement of qualifying documented losses up to $5,000. Choosing $40 means giving up both loss reimbursement and medical monitoring.
The defendants deny wrongdoing. Payments depend on final approval and resolution of appeals. The official website still describes approval as pending; this page does not confirm that final approval has been granted.

Who qualifies
- You must be a living individual in the United States whose private information was potentially compromised in the October 2025 incident. The court case is in Louisiana, but the class is not limited to Louisiana residents.
- The administrator’s notice identifies potential class members. Contact the administrator if you are unsure whether you belong to the class. The incident was identified on October 10, 2025; that is not the incident’s October 5 date.
- Excluded are parents, subsidiaries, directors, officers, members and agents of the defendant, entities it controls, governmental entities, the assigned judge and their immediate family and court staff, valid opt-outs, and people found guilty of or pleading no contest to initiating, causing, aiding or abetting the criminal activity of the incident.
- A valid signed timely claim is required. Submit only one claim per person.
How to file a claim
- 1
Find your settlement notice
Use the Login ID and PIN in your settlement notice for online filing. For help finding them, contact info@RegionalUrologyDataSettlement.com or (833) 200-7004. A paper claim form is available in the official documents; its Login ID field says “if known.”
- 2
Choose one benefit option
Choose $40, or choose medical monitoring and/or documented loss reimbursement up to $5,000. You cannot combine the $40 payment with either of those other benefits.
- 3
Document your losses and sign
For reimbursement, provide records such as receipts or bank statements and explain the connection to the incident. Notes you prepared yourself can support other evidence but are insufficient alone. Complete the contact and payment details, and sign the certification under penalty of perjury.

Frequently asked questions
Can I receive $40 plus medical monitoring?
No. The $40 alternative replaces both medical monitoring and loss reimbursement. Members who do not choose $40 may request medical monitoring, documented loss reimbursement, or both.
What losses qualify for reimbursement?
The losses must be actual, documented, unreimbursed and fairly traceable to the incident. Examples include qualifying fraud or identity-theft losses, credit-report or monitoring fees, identification replacement and postage. The notice describes losses between October 5, 2025 and November 16, 2026; the claim’s expense section specifically asks for losses incurred after October 5 through the date you file. Claim only losses already incurred. Fraud or identity-theft claims also require reasonable efforts to avoid the loss or seek other reimbursement, including applicable insurance. The maximum is $5,000 per person.
Does the $40 option require loss receipts?
No loss documentation or explanation is required for the $40 alternative. You still must be a participating class member and submit a valid signed claim.
What does medical monitoring include?
One year of CyEx Medical Shield Complete includes $1 million of medical identity theft insurance, healthcare insurance ID and medical-record-number monitoring, unauthorized health-savings-account-spending monitoring, and fraud-resolution support. It must be requested and cannot be combined with the $40 alternative.
What does the Medicare question mean?
The claim form contains a Medicare reporting question for people who were Medicare beneficiaries from October 5, 2025 to the present and are seeking reimbursement related to emotional distress. If it applies, the form directs you to contact the administrator for additional reporting information. It does not list a separate automatic emotional-distress payment.
When will payments be issued?
Payments depend on final approval and resolution of any appeals. The notice scheduled a hearing for October 19, 2026, but the reviewed public documents do not confirm its outcome. Check the official site for updates; there is no guaranteed payment date.
Can I still opt out or object?
The notice’s October 16, 2026 deadlines have passed. A person who validly opted out cannot claim benefits. Consult the official notice for the separate procedures and their requirements.
