How we verify settlements

People whose private information was potentially compromised in the June 2023 Valley Oaks Health incident and who appear on the settlement class list may claim benefits by November 9, 2026. Submit a valid claim online or by email, or mail it with a postmark by that date.

Eligible class members may choose a $40 alternative cash payment or claim qualifying losses and time spent responding to the incident. Ordinary losses and lost time share a $500 combined cap; extraordinary losses can qualify for up to $5,000. Two years of medical monitoring may be requested with either cash option.

Valley Oaks denies wrongdoing. The settlement has preliminary approval, and payments depend on final approval and resolution of appeals. The final approval hearing is scheduled for December 10, 2026 at 2 p.m. Eastern Time.

Who qualifies

  • Your private information was potentially compromised in the Valley Oaks incident between June 8 and June 13, 2023, and you are included on the settlement class list. Being a Valley Oaks patient or hearing about the incident alone does not establish eligibility.
  • The class definition does not impose a separate Indiana residency requirement. If you are unsure whether you are on the list, contact the administrator using the official contact details.
  • Excluded are the judge evaluating the settlement, court personnel and their immediate families; Valley Oaks and the related entities and representatives listed in the notice; and people convicted of, or pleading no contest to, specified criminal involvement in the incident.
  • You cannot receive benefits if you validly opted out. A valid timely claim is required for payments and medical monitoring.

How to file a claim

  1. 1

Find your notice and login information

Online filing requires the Login ID and PIN from your settlement notice. If you cannot locate them or are unsure about class membership, contact info@ValleyOaksDataSettlement.com or (888) 836-1354. The paper form asks for a Login ID if known.

  2. 2

    Choose the appropriate cash option

Choose either the $40 alternative payment or the qualifying ordinary-loss, extraordinary-loss and lost-time benefits. You cannot add the $40 alternative to those other cash benefits. Medical monitoring can be selected with either option.

  3. 3

Gather proof and complete the form

Loss claims need supporting records such as receipts and an explanation connecting them to the incident. Self-prepared notes alone are insufficient. For lost time, describe the tasks and hours spent. Supply contact and payment details, then sign and date the certification under penalty of perjury.

  4. 4

    Submit by November 9

Submit online or email an electronic image of the completed claim to info@ValleyOaksDataSettlement.com by November 9, 2026. Alternatively, mail it through USPS to Valley Oaks Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799, postmarked by November 9. The reviewed instructions do not state an exact cutoff time or timezone. Submit only one claim per person.

Frequently asked questions

Can I claim $500 for ordinary losses plus $80 for my time?

No. Ordinary losses and lost time share a combined $500 cap. Lost time is limited to four hours at $20 per hour, for up to $80, and requires a brief description of the incident-related actions and time spent. The time benefit may be claimed without a documented monetary loss, but it cannot be combined with the $40 alternative payment.

What qualifies for extraordinary-loss reimbursement?

You must document an actual unreimbursed monetary loss more likely than not caused by the incident, not already covered as an ordinary loss. The issued claim form covers losses from June 8, 2023 through November 9, 2026. You must have made reasonable efforts to avoid the loss or obtain reimbursement, including using applicable insurance or protection benefits. Approved extraordinary-loss reimbursement cannot exceed $5,000 or the documented loss.

What ordinary expenses qualify?

Qualifying actual unreimbursed incident-related expenses include credit reports, monitoring or credit freezes, replacing identification and postage to contact banks. The issued form covers expenses from June 8, 2023 through November 9, 2026. For monitoring and similar mitigation costs, the agreement specifies costs incurred through your claim submission date. Provide supporting records; the administrator determines validity.

Does the $40 option require loss receipts?

No loss documentation or explanation is required for the $40 alternative payment, but you must be a participating class member and submit a valid signed claim. It replaces ordinary-loss, extraordinary-loss and lost-time payments. You may still request medical monitoring.

What if my reimbursement claim is denied or approved for less than $40?

Under paragraph 14 of the agreement, a participating class member’s ordinary-loss, extraordinary-loss or lost-time claim that is denied entirely or approved for less than $40 is treated as a claim for the $40 alternative payment. The administrator still verifies class membership and claim validity.

Is medical monitoring automatic?

No. Select it on the claim form. Eligible members can request two years of CyEx Medical Shield Pro, including medical identity monitoring, fraud-resolution assistance and $1 million of medical identity theft insurance. Monitoring is available regardless of which cash option you choose.

When will payments arrive?

Payments depend on final approval and resolution of appeals. The hearing is scheduled for December 10, 2026 at 2 p.m. Eastern Time. Check the official website for changes; no guaranteed calendar payment date is stated.

Can I opt out or object?

The notice gives November 9, 2026 as the opt-out and objection deadline. These are separate procedures with detailed requirements in the official notice. A valid opt-out means you cannot receive settlement benefits.
