How to file a Southern Graphics settlement claim

  1. 1

    Check your notice and eligibility

Review the class definition and exclusions. The online form requires your LoginID and PIN from the notice. The paper form asks for your Login ID if known. Contact Simpluris through the official site if you need help.

  2. 2

Choose benefits and gather any required records

Choose documented-loss reimbursement or alternate cash, not both. You may also request the two-year monitoring benefit. File online by November 3, 2026 at 11:59 p.m. Eastern Time, or mail the signed form and supporting documents with a postmark no later than November 3 to Southern Graphics Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958. The administrator may request additional information.

Frequently asked questions

How much can I receive?

You may claim up to $3,500 for qualifying documented out-of-pocket losses, or elect an alternate cash payment. The alternate amount is not fixed. Under the settlement agreement, administration, court-approved awards and fees, monitoring and valid documented-loss claims are paid before alternate cash is distributed. Payments can be adjusted pro rata; the maximum is not guaranteed.

What proof is required?

Documented-loss claims require reasonable supporting records, such as receipts, correspondence or telephone records. Qualifying contemporaneous records can be prepared by the claimant or a third party, but personal certifications, declarations or affidavits alone do not establish a loss. Previously reimbursed expenses cannot be claimed. Alternate cash does not require loss documentation, but all claims still require eligibility, completed information and a signed certification under penalty of perjury.

Can I claim monitoring and cash?

Yes. The settlement permits two years of CyEx Financial Shield Complete monitoring in addition to either documented-loss reimbursement or alternate cash. You cannot select both cash options.

When are claims due?

November 3, 2026. Agreement paragraph 65 specifies 11:59 p.m. Eastern Time for online claims. The official notice requires mailed forms, including supporting documentation, to be postmarked by November 3.

When will payments be sent?

No payment date is confirmed. The court must grant final approval and any appeals must be resolved before benefits are distributed. Check the administrator website for updates.

Do I need a ClaimBee subscription to file?

No. Use the official administrator website or paper claim form to file directly. ClaimBee provides this settlement reference; Simpluris administers the settlement.
