Last updated:
Sources last checked:
The $1.3 million DAP Health settlement covers people in the United States whose private information was compromised in the July 2024 data incident. Eligible members may claim up to $5,000 in documented losses, an estimated $25 pro rata payment, and two years of credit and medical record monitoring. Qualifying California residents may also claim a $75 statutory payment. These benefits can be combined when their conditions are met, but amounts and monitoring duration depend on available funds.
The administrator currently accepts claims through October 21, 2026. Submit online by 11:59 p.m. Pacific time, or mail a signed claim postmarked by that date. Online filing requires the Unique ID and PIN from your notice. No payment arrival date is confirmed; distribution depends on court approval and the settlement becoming final. If you received a re-mailed postcard notice, ask the administrator to confirm your individual deadline.

Who qualifies
- You are an individual in the United States whose private information was compromised in the July 2024 DAP Health data incident, including individuals who received notice of the incident. Being a patient alone does not establish that your information was affected.
- The additional California payment requires that you were a California resident on July 22, 2024 and DAP Health mailed you notice of the data incident. This is an additional benefit condition, not a California-only limit on the entire class.
- Excluded are the presiding judges, class counsel and their families; DAP Health and the related entities, current or former officers and directors specified in the agreement; valid opt-outs; and successors or assigns of excluded persons. Read the official notice for the complete exclusions.
How to submit a DAP Health settlement claim
- 1
Find your notice and check eligibility
Confirm that your notice concerns Donald Crosslin and Matthew Paone v. DAP Health, Inc., Case No. CVRI2500186. The online portal requires your Unique ID and PIN. If your notice is missing, contact the DAP Data Incident Settlement Administrator using the official website’s contact instructions.
- 2
Select the benefits you qualify for
You may select documented losses, pro rata cash and monitoring together. Select the California payment only if you meet its residence and incident-notice requirements. Monitoring must be requested; the claim form says instructions and an activation code will be sent to the email address you provide after the settlement is final.
- 3
Attach loss records and sign personally
Documented-loss claims need reasonable supporting records showing the amount, date and connection to this incident. Claimed losses must be unreimbursed and incurred on or after July 22, 2024 through the date you submit your claim. A personal statement or handwritten receipt alone is not sufficient. The agreement requires your own physical or electronic signature under penalty of perjury.

File before the deadline and save confirmation
Submit through the official portal by October 21, 2026 at 11:59 p.m. Pacific time, or mail the signed paper form postmarked by October 21 to DAP Data Incident Settlement Administrator, P.O. Box 3245, Portland, OR 97208-3245. Save your online confirmation or mailing record, respond to administrator requests, and keep your contact details current. ClaimBee is an independent reference, not the administrator. The agreement gives recipients of a re-mailed postcard notice 90 days from its distribution; confirm that deadline directly with the administrator.
Why the listed payments are not guaranteed
The fund also pays administration, court-approved attorneys’ fees and expenses, service awards and monitoring. The agreement prioritizes monitoring before documented losses, then qualifying California payments, then pro rata cash. The final pro rata amount depends on funds left and the number of valid claims.
If available funds cannot cover the selected monitoring, its duration will be shortened and no cash payments will be made. If monitoring and documented losses exhaust available funds, loss payments will be reduced and there will be no California or pro rata cash payments. The estimated $25 and stated $75 must not be treated as guaranteed checks.
Current filing information
The official website’s current-status panel says claim forms are being accepted until October 21, 2026. The claim form and notice use a postmark rule for mailed claims; agreement sections 72 and 113(a) expressly confirm that rule. Section 72 specifies 11:59 p.m. Pacific time for online filing. Section 119(e) separately allows 90 days from distribution for recipients of a re-mailed postcard notice; those members should confirm their individual deadline with the administrator.
The September 1, 2026 opt-out and objection deadlines listed in the notice have passed. Remaining in the settlement may bind class members to its release even if they do not submit a benefit claim. Consult the official materials or your own lawyer for questions about individual rights.

Frequently asked questions
Can I request both loss reimbursement and the estimated $25?
Yes. The official claim form and agreement allow the pro rata cash payment in addition to documented losses. Eligible members may also request monitoring and, if qualified, the California statutory payment. None of the advertised cash amounts is guaranteed; the allocation rules can reduce or eliminate cash benefits if available funds are insufficient.
What documents do I need for a loss claim?
Use records such as bank or credit-card statements, invoices, telephone records, photographs or receipts. They must support actual, unreimbursed losses more likely than not traceable to this incident. Self-prepared notes or your certification alone are insufficient. The paper form covers losses from July 22, 2024 through your claim-submission date.
Who qualifies for the additional California payment?
A settlement class member who lived in California on July 22, 2024 and to whom DAP Health mailed notice of the incident may request the $75 statutory payment. The form includes a residency attestation under penalty of perjury. This payment is subject to the fund-allocation rules.
Do I need loss documents for pro rata cash or monitoring?
No loss documents are required to select the pro rata cash option. Monitoring is also available to eligible class members who request it. You must still complete a valid signed claim, verify class membership, and provide any additional information the administrator requests. Online claims require notice credentials.
How will I receive payment?
The form offers PayPal, Venmo, Zelle, ACH transfer or a mailed check. A check is the default if you do not choose an electronic method; selecting multiple payment methods also results in a check. Provide payment information only through the official administrator’s process. Physical checks must be cashed within 90 days of issuance under the agreement.
When will benefits be distributed?
No exact distribution date is confirmed. The official materials make distribution conditional on court approval and the settlement becoming final. The website lists an October 1, 2026 approval hearing, but the materials reviewed do not establish its outcome. A hearing date is not a promised payday.
What if my notice was re-mailed?
Agreement section 119(e) gives individuals who receive a re-mailed postcard notice 90 days from the distribution date of that re-mailed notice. Ask the administrator to confirm the date that applies to you. Do not assume that simply losing your notice or requesting a duplicate automatically extends the general October 21 deadline.
