Who qualifies
- The court’s class covers individuals and law firms, including solo practitioners, who requested records from a Maryland healthcare provider or authorized someone else to request their records. An eligible Ciox fee must have been charged and paid between October 1, 2016 and July 11, 2025. The location of the healthcare provider matters.
- Law firms must have paid the fee on behalf of an individual and remained unreimbursed. Eligible charges include any fee charged after Ciox determined no records existed; fees for records produced above the annually adjusted per-page rates or aggregate caps in Maryland Health-General § 4-304(c)(3)(i) and (iii)(2); and retrieval fees, sales tax, electronic delivery fees or certification fees.
- The settlement includes certain copying services and insurers in the law-firm category only when they requested records with the individual’s authorization for a copying or insurance matter and paid an unreimbursed No Records Found fee. An attorney need not have been involved in that request.
- The current online form requires the filer to be associated with a listed organization that received notice. An individual can qualify if they directly paid an eligible fee or reimbursed someone who paid it on their behalf. Individual claims begin with the separate Phase II notice.
- The court excludes current and former Ciox executives and their spouses, parents, siblings and children, Ciox employees, officers and directors, and court employees. People who properly exclude themselves from the settlement do not receive a payment.
How organizations can file a Ciox Maryland claim
- 1
Check the notice and each invoice
Find your organization’s verification code and PIN for the online form. Match each eligible fee to its medical-records request. Several qualifying fees on a single invoice count as one claim. Contact Strategic Claims Services at 866-274-4004 if you are unsure whether your organization is on the class list.
- 2
Confirm who paid
Provide the required name, mailing address, email and phone number. Identify eligible transactions and affirm that your organization paid the fee and did not receive reimbursement. A law firm may provide contact information for clients who reimbursed it so those clients can receive notice. The public form describes this step as encouraged.
- 3
Submit by December 4, 2026
File online through the official organization claim form, or send the completed form by prepaid first-class U.S. mail postmarked by December 4 to Ayers, et al. v. CIOX Health, LLC Class Action Settlement, c/o Strategic Claims Services, P.O. Box 230, 600 N Jackson St, Suite 205, Media, PA 19063. The administrator can mail you a paper form. Keep your confirmation or mailing record.
- 4
Respond to verification requests
The administrator reviews claims for eligibility, completeness and duplicate invoices. A copying service must provide requested supporting documentation within 15 days or its affected claims will be disallowed. If an individual and a law firm claim the same transaction, the agreement presumes the individual paid unless the law firm establishes otherwise.

Frequently asked questions
How much might an approved claim receive?
The initial allocation is $45.76 per approved invoice. The administrator reduces payments proportionately if available funds are insufficient. If funds remain, part can go toward administration costs under the agreement and the remaining amount can increase payments. Attorneys’ fees and expenses require court approval. No fixed amount is guaranteed.
Can my organization claim multiple invoices?
Yes. It may claim separately for eligible unreimbursed fees on distinct medical-records requests, with at most one approved claim for each invoice. It may not claim a fee a client reimbursed.
When can individuals file?
Individuals have their own Phase II notice and may file starting on that notice date. During their phase, individuals may file through the official website, by paper mail or by fax. For each invoice they claim, they must affirm they paid the eligible fee or reimbursed someone who paid it on their behalf. The signed agreement sets their deadline within 60 days after final approval. The long-form notice lists May 22, 2027 or 60 days after final approval, and the fairness hearing is currently scheduled for March 23, 2027. The court has not yet established an actual final approval date. Contact the administrator about the individual form and current timing. The December 4, 2026 deadline and current online form apply to organizations.
When will payments be sent?
The settlement must receive final court approval and become effective. The notice describes payments after the later of the effective date and the claims deadline, with a 45-day payment period. Appeals may delay payment.
What rights are affected?
Class members who remain in the settlement release covered medical-records fee claims against Ciox and the healthcare providers it represented, even if they do not file for payment. Review the long-form notice and agreement for the full release and separate deadlines.
